Complete the application form (Word). The application is a fillable form. Answer all questions on the application in the spaces provided. Do not exceed the required space allotment or attach pages unless specifically directed to do so.

Deliver four complete copies of the Preliminary Application plus one complete electronic version on CD to DCR no later than 4 p.m. on March 19, 2012, to 203 Governor Street, Suite 326, Richmond, Virginia 23219. Postmarks, faxes and email submissions will not be accepted. Applicants are responsible for effecting delivery by the deadline above; late submissions will be rejected without consideration.
